When you give your details for the Covid Vaccination certificate, better to choose Passport number as your preferred id & give your expanded name as exactly in your passport.
It will be helpful for Overseas travel.
And without fail, please take acknowledgement and keep it with you. In future, the health insurance companies will insist on the same to cover corona hospitalisation or hike premium for those not having a record of vaccination.
The 14 digit number is necessary.
The vaccination certificate will be another attachment to passport for international travel and it will be mandatory for inter state travel also. It will be mandatory for insurance, for hospital admissions and surgeries and what not?
Similar to HIV test before any surgery.
