失智症（英語：Dementia、德語：Demenz），也稱癡呆症，其英文Dementia一字來自拉丁語（de-意指「遠離」 + mens意指「心智」）。是腦部疾病的其中一類，此症導致思考能力和記憶力長期而逐漸地退化，並使個人日常生活功能受到影響。大部分失智症都是退化性的。亦有腦血管疾病所引起的失智症。且只有不到5%的失智症其成因是可以治療的。最常見之阿茲海默症的真正成因至今仍然不明。
